How to apply for a Home Care Package

You have taken the first step towards ensuring you have the help you need!

You can request an assessment through my aged care for assistance to help you remain at home.
There are a few ways this can be done.

• Call 1800 200 422 and advise them you would like an assessment for a home care package to assist you with daily living tasks.
• An online referral which a GP / medical professional can also complete on your behalf.
• You or a loved one can complete the online referral directly through this link.

Make a Referral | My Aged Care

They will ask you questions about your health, support system, medical diagnosis and what you struggle with on a day-to-day basis. Following this initial referral, an assessor from my aged care will contact you on the phone to ask you further questions which determine what level of care you require. If deemed to have higher care needs, you will be assessed by an aged care assessment team member (ACAT) for a home care package. The assessor will determine the level you are eligible for and the priority for the package to be assigned.

Do I have to pay any fees?

All providers will charge a fee to cover their services, these fees are deducted directly from the package and not out of pocket to you.

Fees that may apply to you are

• Basic daily fee – a contribution that is determined by the government that is not mandated. Jesmond At Home can waive this fee accordingly.
• An income tested care fee – is determined by an income assessment to be completed by you with Services Australia. This fee depends on your income and assets. Your provider is liable to charge you this fee to be put towards your care and services.
• Extra charges for additional care and services as agreed with your provider.
